I have been trying to complete the form to establish a monthly service. Unable to do so because your format will not let me insert "initials" where needed and consequently I cannot proceed to the section that requires my signature. I tried to download the form so I could fill it out and mail it in but no luck there either. Someone needs to send or email me a copy that can be downloaded so I can fill it out and send it in. Otherwise I'll have to find another service.
